An Interview with Dr. Ashwinikumar Aiyangar

Dr. Ashwinikumar Aiyangar is a senior Consultant Nephrologist at Apollo Hospitals, Secunderabad, an endurance athlete, musician, and now a debut novelist. With more than a decade of experience in nephrology, dialysis, and kidney transplantation, he has translated the emotions, sacrifices, and humanity of medical practice into his first novel, White Coat, Red Heart: The Journey of a Girl, a Doctor, a Healer. In this conversation, he shares the inspiration behind his book, his journey as a doctor and author, and why empathy remains the heart of healthcare.

Q1. After years in medicine, what inspired you to write your first novel?

Medicine has given me countless stories that never appear in medical textbooks. Every patient, every family, every difficult decision leaves an emotional imprint. I always wanted to write about the people behind the white coats; their fears, hopes, failures, and triumphs. White Coat, Red Heart became my way of showing that doctors are not just professionals; they are human beings navigating extraordinary situations with compassion and resilience.

Q2. Tell us about White Coat, Red Heart.

The novel follows Dr. Kavya Rao, a young doctor whose journey begins as an uncertain medical student and gradually evolves into that of a compassionate healer. Set within the demanding environment of hospitals, the story explores the balance between science and empathy, ambition and peace, intellect and humanity. It is not merely a medical novel; it is a story about personal growth, relationships, and discovering that healing often extends beyond medicine.

Q3. How much of the book is inspired by your own experiences?

Although the novel is fiction, many emotions, situations, and observations are drawn from real life. Having spent years caring for patients with kidney disease, dialysis, and organ transplantation, I have witnessed moments of immense courage, heartbreak, gratitude, and hope. These experiences naturally influenced the emotional authenticity of the narrative.

Q4. Your profession deals with life-and-death situations every day. How has that shaped you as a writer?

Medicine teaches humility. Every patient reminds us that life is fragile and precious. As doctors, we celebrate recoveries but also learn to cope with loss. These experiences make one more empathetic, and empathy is the foundation of meaningful storytelling. I wanted readers to understand that medicine is not only about diagnosis and treatment; it is equally about listening, comforting, and standing beside people during their most vulnerable moments.

Q5. The title White Coat, Red Heart is quite evocative. What does it signify?

The white coat represents science, discipline, and professional responsibility. The red heart symbolises compassion, kindness, courage, and the emotional core that every healthcare professional must preserve. My belief is that technical excellence alone is never enough; true healing begins when medicine is practised with humanity.

Q6. As a nephrologist, what lessons have your patients taught you?

Patients teach resilience. Many live with chronic illnesses that require lifelong treatment, yet they continue to inspire those around them with remarkable strength and optimism. Their courage constantly reminds me that healing is not only physical: it is also emotional, psychological, and spiritual.

Q7. Apart from medicine, you’re also an Ironman triathlete, marathon runner, and musician. How do these passions influence your work?

Endurance sports teach patience, discipline, and consistency. Music teaches rhythm, creativity, and emotional expression. Both have helped me become a better doctor and writer. Whether running a marathon, performing music, or treating patients, success comes from persistence, balance, and genuine passion.

Q8. What message would you like readers to take away from your novel?

I hope readers realise that behind every hospital corridor, every consultation, and every white coat is a human story. Doctors and patients are connected not just by illness, but by trust, hope, and compassion. If readers finish the book with greater empathy for healthcare professionals and patients alike, I will feel that the novel has achieved its purpose.

Q9. Finally, how would you describe yourself beyond your professional achievements?

I see myself as someone who believes in lifelong learning. Whether I am treating patients, writing stories, running long distances, or playing music, my goal remains the same—to keep growing while making a positive difference in people’s lives. At the heart of everything I do is the belief that compassion is our greatest strength.
